We provide the following in home services to clients ages birth to 21 who are MaineCare eligible recipients:

• Targeted Case Management Services--we specialize in medical TCM, but also provide behavioral & cognitive TCM services.
  • TCM services help families connect to resources for food, housing, school, DME needs, coordination of providers, etc. All TCM services are currently being completed by Telehealth.

• Pediatric Block Time Nursing and Personal Care services--allows for a RN or PSS to provide direct in home care.
  • Providing needed medical and personal care to help keep kids out of alternative settings during this time.
• Vineland Assessments-completed via Telehealth
